Referrals are accepted directly from community professionals, parents/guardians or adult clients. Please email our Administration Manager, Silvester Pushpanathan ([email protected]) or call, 519-936-0108 if you have any questions about our services. Given our high volume of service inquiries, only email inquiries can be promptly returned. Telephone inquiries are responded to within 10 days.
To make a referral, please email [email protected]. You will be sent an electronic referral form which is then securely submitted to our server. If you are unable to complete a registration form on your computer or device, please contact [email protected] and Silvester, will ensure that you receive a hard copy. Clients are put on our wait list for service once a referral has been completed.
After receiving your intake form, our Clinical Practice Manager, Michelle McIntyre, will contact you to confirm that our services are a good fit for you or your child’s needs, to let you know when our services may be available, and to book an initial intake session.
Child and youth clients usually do not attend the intake appointment given that sensitive issues are often discussed. Parents or guardians are asked to advise us if they feel it is in the child or youth client’s best interests to attend the intake appointment.
We regret that we are unable to offer parking in the back lot of 370 Queens Ave as our staff use our allocated parking. Unless clients have physical disabilities or have children in infant carriers, we ask you NOT to use visitor spots. Metered parking is available both north and south of Queens Ave on Waterloo St.
